Joining Our Practice

Step 1 - Know that our office visits are billed to insurance and require copays

Step 2 - Book a Visit Online or by calling 882-6468 (MINT)

Choose from a Wellness Physical, which is typically fully covered by insurance or a New Patient Visit to address one or two specific health concerns.

Step 3 - You’ll receive a link to new patient forms where we ask you to select a membership.

Step 4 - Take a deep breath, you now have the best primary care in Vermont!

Driving directions: Turn into the Congregational Church parking lot and continue to the Mint Health parking signs. Enter through the rear door and relax in our cozy waiting room.
