Joining Our Practice

We are a member-supported primary care practice based in Waterbury, VT offering simple and easy access to healthcare.

The best way to join is by booking either a New Patient Visit or a Wellness Physical directly on our website.

  • Wellness Physicals are annual check-ups for general health maintenance, typically fully covered by insurance.

  • New Patient Visits are to address one or two specific health concerns in addition to reviewing your health history. These appointments may involve a copay and are often subject to your deductible.

Once booked, we will send you:

  • Patient Intake Forms: Look for an email from Mint Health / Zentake.io

  • Patient Portal Link: A link to our patient portal, called Passport.

  • Membership Bill: We will send you a bill for your membership based on your preferences indicated in the intake forms and your first visit. You may want to contact your HSA administrator to determine if concierge fees are eligible.

  • Enter our parking lot between 6 and 8 N Main St, just past the white Congregational Church and park in the rear of the building. The parking lot operator will put branded signs up soon for us, but patients are welcome to park for free in the interim.

  • Enter through the rear entrance and relax in our cozy waiting room.

  • After your visit, feel free to call, text, or portal message us anytime you have a health concern arise. Your provider will get back to you quickly with a plan.
